Clara has just mentioned seeing River. The Doctor says he always sees her.
When Clara says she saw her, The Doctor is hit with the realisation that River in fact is real, and not just in his imagination. And that kills him.
He’s crying because he thought he had an ending with her, but now he has to have another.HE’S CRYING BECAUSE SHE’S REAL.
